All information collected on the Health History form, during the intake process, subsequent sessions, and through email correspondence are private and confidential. Information will only be shared with the written permission of the client.
I request at least 24 hours notice if you would like to change or cancel an appointment. After 24 hours, you will be charged the appointment fee for the missed or canceled appointment.
Please note that the services are being offered by a Registered Massage Therapist and therefore, the services provided are professional and treatment-based. Unprofessional client conduct will result in immediate termination of the treatment and full payment of the appointment booking will be charged.
